Effective Date: September 1, 2026
This Notice describes how medical and other protected health information about you may be used or disclosed by Sun Care Medical Supply of Central Florida LLC ("Sun Care Medical Supply," "Sun Care," "we," "our," or "us"), and how you may access this information. Please review this Notice carefully. Sun Care Medical Supply is committed to protecting the privacy and security of your Protected Health Information ("PHI") in accordance with the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), applicable federal regulations, and other applicable laws.
1. Your rights
Obtain a Copy of Your Health Information
You may ask to inspect or receive an electronic or paper copy of the health information we maintain about you, including records related to your medical equipment, supplies, prescriptions, orders, billing, and services. We will generally provide access within the time required by law. We may charge a reasonable, cost-based fee when permitted.
Ask Us to Correct Your Information
If you believe information in your record is incorrect or incomplete, you may ask us to amend it. We may deny the request in certain circumstances permitted by law. If we deny your request, we will provide an explanation in writing and explain any additional rights you may have.
Request Confidential Communications
You may ask us to communicate with you in a particular manner or at a particular location. For example, you may request that we call a specific telephone number, contact you only at certain times, send correspondence to a different mailing address, or use another reasonable method of communication. We will accommodate reasonable requests.
Request Restrictions
You may ask us not to use or disclose certain health information for treatment, payment, or health care operations. We are generally not required to agree to a requested restriction unless the law requires us to do so. If you pay for an item or service completely out of pocket, you may ask us not to disclose information concerning that item or service to your health plan for payment or health care operations. We will honor the request unless disclosure is required by law.
Receive an Accounting of Certain Disclosures
You may ask us for a list of certain disclosures of your health information made during the six years before your request. The accounting generally does not include disclosures made for treatment, payment, health care operations, disclosures you authorized, or certain other disclosures excluded by law. One accounting during a 12-month period will be provided without charge. We may charge a reasonable cost-based fee for additional requests during the same period.
Receive a Copy of This Notice
You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.
Choose Someone to Act for You
If you have given another individual legal authority to make health care decisions or otherwise act on your behalf, such as through a medical power of attorney or legal guardianship, that individual may exercise your privacy rights when permitted by law. We may request documentation verifying that person's authority.
File a Privacy Complaint
You may file a complaint if you believe your privacy rights have been violated. Sun Care Medical Supply will not retaliate against you for filing a privacy complaint.
2. Your choices
Family Members, Caregivers, and Others Involved in Your Care
With your permission, or when otherwise allowed by law, we may share relevant information with family members, personal caregivers, authorized representatives, home health providers, facility staff, case managers, or others involved in your care or payment for your care. If you are unable to communicate your preference, we may disclose limited information when we reasonably believe the disclosure is in your best interest or is necessary to protect your health or safety.
Disaster or Emergency Situations
We may share limited information with organizations assisting with disaster relief or emergency response when permitted by law.
Marketing and Sale of Information
We will obtain your written authorization before using or disclosing your health information for marketing purposes when HIPAA requires authorization. We will not sell your Protected Health Information without your written authorization except where specifically permitted by law.
Fundraising
If we engage in fundraising activities involving PHI, you will have the right to tell us not to contact you for fundraising purposes.

Examples may include billing services, software and electronic health record providers, secure document storage, information technology services, accounting services, legal services, equipment servicing or fulfillment companies, and other vendors supporting our operations. If Sun Care Medical Supply receives or maintains patient records that are protected by the federal confidentiality requirements applicable to substance use disorder records under 42 CFR Part 2, additional protections may apply. When those protections apply, such records generally may not be used or disclosed in civil, criminal, administrative, or legislative proceedings against you based on the content of those records unless the requirements of applicable federal law are satisfied, such as obtaining your written consent or an appropriate court order and subpoena. Sun Care Medical Supply will comply with applicable federal requirements governing these records.
